If you were setup at a front desk and were given a temporary password on paper, please review these tips:

The temporary password is case sensitive (capitalization matters).

The numeral one (1), capital I (as in igloo), and lowercase l (as in lake) look very similar on paper.  Try substituting these three characters in your temporary password.

The numeral zero (0) and a capital O (as in Oxford) look very similar on paper.  Try substituting these characters in your temporary password.

After three failed login attempts, for security reasons, you will be locked out of ‘My NWPC Chart’ for 30 minutes.  Please wait 30 minutes and try again

 

 

If you were given a PIN letter instead of a temporary password, please review these tips.

The numeral one (1), capital I (as in igloo), and lowercase l (as in lake) look very similar on paper.  Try substituting these three characters in your PIN.

The numeral zero (0) and a capital O (as in Oxford) look very similar on paper.  Try substituting these characters in your PIN.

Please spell your name exactly as it appears on the PIN letter.  This includes misspellings on your name.  Misspelled names can be corrected after ‘My NWPC Chart’ is setup.  Please visit the demographics page to submit your name correction.

If you have forgotten your password?

You have two options for resetting your password. 

  1. Use the ‘I forgot my password’ link on the ‘My NWPC Chart’ login page.

To use the ‘I forgot my password’ link simply click the link from the ‘My NWPC Chart’ login page or navigate directly there using THIS link.  Provide your username or email that is associated with your account and select Reset Password.  You will need to then answer the two hint questions that you set up the first time you logged into your ‘My NWPC Chart’ account.  Two emails will be sent to the email address associated with your ‘My NWPC Chart’ account.  The first email is to inform you that your password was reset. The second email will contain your new temporary password.  Use this password to log into your ‘My NWPC Chart’ account.  This password is case sensitive and must be typed exactly as it appears in your email.  Upper case and lower case letters are important. You will be promted to change your password when you log in the next time.

     2.  Reset your password at the front desk during your next visit.

 

If you were emailed a temporary password either by answering your hint questions or by speaking with a front desk staff member

 

You will receive two emails.  The first one will let you know that your password was changed.  The second email will contain the new temporary password.

The temporary password is case sensitive (capitalization matters).

If you choose to copy and paste the password (recommended) be careful not to select the space after the password.  The space character will get copied along with the rest of the password and cause your password to not work.

 

If you would like to change your email address 

 

There are two options for changing your email address. 


1. Change your email online.  Log into your ‘My NWPC Chart’ account and click the link to ‘Edit John Doe’s Account’ located above the NWPC logo.  From here you can edit almost all of your ‘My NWPC Chart’ information,  including your email address.

2. Change your email at the front desk of a NWPC clinic.  To change your email address in person at a front desk present photo ID to a NWPC front desk staff member and ask for a ‘My NWPC Chart Email Address Change Form.’  Fill out the form and return to the front desk staff.   In addition to changing your email address this form also has the option to have your password reset if you have forgotten it.  Expect your email address change to be processed in two business days.

 

Can I register over the phone or without visiting a clinic?

We take our patients medical privacy very seriously. To sign up for ‘My NWPC Chart’ you must visit a clinic in person with your personal identification.
